Steps in Joining the ASH Fitness Network

1. Joining the Fitness Network

  • Club submits the fitness application to ASH Networks.
  • Once the application is received at ASH Networks it is processed and will take up to 3-4 weeks to become active.
  • Once club becomes active a welcome packet is sent to the contracted club.
  • Club location and amenity information is posted on various ASH Networks websites.

2. Member Responsibilities and Confirming Member Eligibility

Member Responsibilities

  • The health plan will outreach to all eligible members to notify them of their eligibility to participate in the ASH Networks fitness program.
  • Member contacts ASH Networks to enroll in the fitness program and selects a primary facility to utilize the fitness benefit.

Confirming Member Eligibility

When an eligible Silver&Fit or Active&Fit member inquires about the program please proceed with the following:

  • Ask the member if they have enrolled in the Silver&Fit or Active&Fit program. They can enroll by calling 877.427.4788 option 2 or by accessing the ASH Client’s Web site or the applicable ASH Networks Web site.
  • If a member states they have already enrolled refer to the ASH Networks membership roster to verify enrollment. The membership roster is your first point of reference to verify member’s enrollment.
  • If member is not on the membership roster call 877.329.2746 option 1 to verify member enrollment.
  • When a member enrolls they will choose your fitness club (members can only choose one club at a time).
  • Member will receive an identification card from ASH Networks approximately 5-10 days after enrolling.

3. Members First Visit, Tracking and Reporting:

Members first visit to the club

  • Sign member up on standard contract (zero down)
  • Fitness club will not charge an initiation fee or membership fee.
  • Issue the member your standard membership card.

Tracking

  • Upon each visit, the member must present or swipe their membership card. If you have members manually “sign in” please use the Member Sign-In Sheet that will be provided in your welcome packet. You will be required to submit this for compensation.

Reporting

  • Fitness Club runs monthly report on member utilization
  • Information to gather for tracking purposes:
    • Members name
    • Members birthdate (month and day)
    • Members health plan, Silver&Fit or Active&Fit identification number
    • Members workout dates
  • Fitness club will submit a billing report to ASH Networks for compensation after 1 month of utilization.
  • Reports may be submitted via ASHLink, email, mail or fax.
